Our formulary is a guide for the selection of medications that are clinically and therapeutically appropriate. All drugs listed have been reviewed by a National Pharmacy and Therapeutics (P&T) Committee and are approved for inclusion.

We require prior authorization for certain drugs. If you do not get prior approval, we may not cover the drug.
In some cases, we require members to try one drug to treat a medical condition before we will cover another drug for that condition.
For certain drugs, we limit the amount of the drug that we will cover per prescription, or we will only provide coverage for a defined period of time.
Specialty drugs used to treat complex conditions often require special handling, specific administration methods, or compliance with FDA-mandated protocols.
To make preventative medications as easy and affordable as possible, we do not require anyone to meet a deductible for these prescriptions.
FORMULARY
Our formulary lists all of the brand-name and generic drugs we cover. It lists drugs found on the preferred drug list (PDL) as well as other drugs.
