Rightway's Care Complete Diabetes program makes managing diabetes easier by providing expert guidance and dedicated support.
Work with an expert care team.
You’ll work with a care team made up of a nurse, pharmacist, and dietitian who specialize in diabetes management and tailor their support and recommendations to your specific needs. No more sitting on hold with your doctor’s office or juggling multiple patient portals. Your entire care team is accessible through one app.
Regular check-ins.
Every quarter, you'll meet with your nurse to review your diabetes testing results, assess any side effects, and evaluate your medication effectiveness. If necessary, they'll connect you with one of our dietitians or pharmacists to help you overcome any challenges you're experiencing.
Seamless digital support.
Stay connected through the same Rightway app you already use to manage your pharmacy benefits.
Members in the program receive proactive care to prevent potential issues. This proactive approach leads to improved medication adherence, better blood sugar and A1C levels, and a decreased risk of hospitalization.
Enrolled members benefit from $0 co-pays on diabetes medications and testing supplies.
Wondering about your dose or how to manage side effects? Your dedicated care team is always here to guide you, so you never have to go it alone.
Chat with your care team and get helpful reminders—right from your phone.
Please note: Your care team does not replace your doctor. They will provide support and coordinate care with your doctor when needed.
Your pharmacist is your first point of contact and remains your resource for medication expertise. They can ensure that your treatment plan is safe and effective.
Your pharmacist creates practical medication schedules tailored to your daily routine, helps troubleshoot side effects, identifies opportunities to optimize your therapy, and coordinates with your healthcare team to ensure all aspects of your medication management support your diabetes goals.
Once you establish care with your pharmacist, your nurse becomes your primary point of contact throughout the program. An expert in diabetes management, your nurse will guide you, conduct quarterly check-ins, and coordinate additional support from the pharmacist and dietitian as needed.
They’re here to answer your questions and help you stay on track with your health goals.
A registered dietitian will assess your lifestyle and work with you to develop a personalized nutrition and exercise plan. They can suggest dietary changes to help manage GI symptoms like nausea and bloating.
They will regularly review your progress and make adjustments to achieve your wellness goals. They will cover eating habits, exercise routines, energy levels, food anxieties, and digestive health.
This program is for Rightway members who are prescribed diabetes medications or testing supplies and are covered by an eligible plan sponsor. Our pharmacy team can confirm your eligibility via chat in our app or over the phone.
